INVITATION TO BID FOR THE PROCUREMENT OF VARIOUS MEDICINES FOR CITY HEALTH OFFICE,
ALAMINOS CITY, PANGASINAN
The CITY GOVERNMENT OF ALAMINOS, PANGASINAN, through its 20% Development Fund intends to apply the
sum of Four Hundred Seventy Two Thousand Four Hundred Eighty Two Pesos and Fifty Centavos
(Php 472,482.50) being the Approved Budget for the Contract (ABC) to payment under the contract for the
PROCUREMENT OF VARIOUS MEDICINES FOR CITY HEALTH OFFICE, ALAMINOS CITY, PANGASINAN.

Alluminum magnesium HCI tabs ( bx of 100 tb) Ascorbic acid 500mg tab ( bx of 100 tab) Chloramphenicol 500mg cap (bx of 100 cap) Chloramphenamine maleate (bx of 100 tab) Diclofenac sodium 50mg tab (bx of 100 tab) Ferrous sulfate 325mg tab ( bx of 100 tab) Mefenamic acid 500mg cap (bx of 100 cap) Metronidazole 500mg tab ( bx of 100 tab) Dental needles small (bx of 100 needles) Phenoxymethyl penicillin 500mg cap (bx of 100 cap) Bids received in excess of the ABC shall be automatically rejected at bid opening. The CITY GOVERNMENT OF ALAMINOS, PANGASINAN now invites bids for the PROCUREMENT OF
VARIOUS MEDICINES FOR CITY HEALTH OFFICE, ALAMINOS CITY, PANGASINAN. Delivery of the Goods is
required within fifteen (15) calendar days upon receipt of the Notice of Award. Bidders should have completed,
within two (2) years from the date of submission and receipt of bids, a contract similar to the Project. The description
of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders.
Bidding will be conducted through open competitive bidding procedures using non-discretionary pass/fail criterion as specified in the Implementing Rules and Regulations (IRR) of Republic Act 9184 (RA 9184), otherwise known as the “Government Procurement Reform Act”. Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines. Interested bidders may obtain further information from CITY GOVERNMENT OF ALAMINOS, PANGASINAN and
inspect the Bidding Documents at the address given below from February 5 – February 24, 2014.
The CITY GOVERNMENT OF ALAMINOS, PANGASINAN will hold a Pre-Bid Conference on February 12, 2014 at
9:30 A.M at BAC Office, 2nd Floor , City Hall Building, Alaminos City, Pangasinan, which shall be open only to
all interested parties who have purchased the Bidding Documents.
A complete set of Bidding Documents may be purchased by interested Bidders from the address below and upon
payment of a nonrefundable fee for the Bidding Documents in the amount of Four Hundred Seventy Two Pesos
(Php 472.00)
Bids must be delivered to the Budget Office, City Hall Building, Alaminos City, Pangasinan on or before February 24,
2014 at 9:00 A.M. at Budget Office, 2nd Floor, City Hall Building, Alaminos City, Pangasinan. All bids must be
accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause 18.a
Bid opening shall be on February 24, 2014 at 9:30AM at BAC Office, City Hall Building, Alaminos City,
Pangasinan. Bids will be opened in the presence of the bidders’ representatives who choose to attend at the address
below. Late bids shall not be accepted.
The bid security shall be in the following amount: The amount of Nine Thousand Four Hundred Forty Nine Pesos and 65/100 (Php 9,449.65) or [ 2% of
ABC], if bid security is in cash, cashier’s/manager’s check, bank draft/guarantee or irrevocable letter of
credit;
The amount of Twenty Three Thousand Six Hundred Twenty Four Pesos and 13/100 (Php 23,624.13)
or [5% of ABC] if bid security is in Surety Bond; or
Any combination of the foregoing proportionate to the share of form with respect to total amount of security. A warranty security covered by either retention money in an equivalent to at least ten percent (10%) of every progress payment, or a special bank guarantee equivalent to at least ten percent (10%) of the total contract price shall be required from the contract awardee for a minimum period of three (3) months, in case of Expendable Supplies or minimum of one (1) year, in case of Non-expendable Supplies, after acceptance by the procuring entity of the delivered supplies. The said amount shall only be released after the lapse of warranty period. The winning bidder shall be paid by the City only after the issuance of Acceptance Report by the City. The CITY GOVERNMENT OF ALAMINOS, PANGASINAN reserves the right to accept or reject any bid, to annul
the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability
to the affected bidder or bidders.
