1. If over age 17, and not a permanent Canadian resident for at least 1 year, obtain a Blood Chemistry
Profile with hepatitis B and C screens in all cases.
2. When Guaranteed Insurability Benefit (GIB) is requested, add 2 times the GIB amount to the Initial Sum
Insured and refer to the table for requirements.
3. If a client is applying for both a Preferred and a Non-Preferred product simultaneously, use the
Preferred products chart for the total Sum Insured applied for.
4. Additional requirements may be ordered at Standard Life’s discretion.
5. For Life Insurance, if a proposed insured has applied for other insurance in the last 12 months with
Standard Life or with another Insurance Company, the total sum insured applied for during this period will be added together to determine the necessary underwriting requirements.
6. An Insured for Critical Illness insurance will be deemed a “non-smoker” if he/she has not, within the
previous 12 months of applying for Critical Illness insurance, used tobacco in any form including: cigarettes, cigarillos, any form of cigars, marijuana, chewing tobacco, and pipe or used any products to stop smoking such as: ZYBAN®, nicotine patches, nicotine gum.
7. APSs will always be required on applicants who have had check-ups within six months of applying for
Critical Illness coverage, or at Standard Life’s discretion.
8. If a client is applying for both Critical Illness and Life Insurance, use the appropriate underwriting
chart for each product and Sum Insured. Do not duplicate the requirements, instead order the most stringent test. (MEDILIFE replaces PARALIFE, stress ECG replaces an ECG etc.)
